Cancellation/ Late Cancellation/ No Show Appointment Policy

I take great pride in choosing a time for your appointment that is convenient to you and appropriate for my schedule, that time is reserved for you.  If you have to cancel your appointment, I expect a 24-hour notice, if you fail to cancel within 24-hour you will be responsible for the late cancellation or no-show fee. 

 If you missed your appointment due to an emergency (illness, ER visit, etc.) please provide verification to waive the fee.  It is the patient’s responsibility to pay the full fee.  Per my office policy failure to cancel appointments without 24-hour notice will be charged at the providers’ hourly rate.  The cancellation fee must be paid before the next appointment.

 No-Show Fees:  Anytime you fail to attend a scheduled appointment without giving appropriate prior notice of cancellation, you will be charged the at the hourly rate.

Late Cancellation Fees:  Any session missed by canceling less than 24 hours in advance will be charged at the hourly rate. 

The credit card information or other payment information you provided will be used to process payments. By providing your credit card information or booking an appointment, you consent to this policy. Multiple no-shows will result in termination.

Ataraxy, Mental Health and Wellness. , is a self-pay service. All services are provided virtually (telehealth).

‍Flexible spending accounts (FSA) and health savings accounts (HSA) allow you to set aside money before taxes, which can be used to pay for your appointments with us.

Debit/ Credit Card, Visa, Mastercard

 All sessions are conducted through telehealth (HIPPA compliance).
